Immunizations

All students must provide up-to-date immunization documentation.

All students must provide proof of all required immunizations for the State of Maine to Baxter Academy prior to attending school. A pdf copy can be uploaded to the Document Uploads section when filling out your online registration. Physical copies can be dropped off at the Front Office, or they can be mailed, emailed, or faxed to the address below:

The student's immunization record must include the following information:

1. The child's name and date of birth

2. A minimum of:

  • 5 DTaP (diphtheria, tetanus, and pertussis) (4 DTaP if 4th is given on or after 4th birthday)

  • 4 Polio (if 4 doses are given before the 4th birthday, an additional dose of age-appropriate IPV should be given on or after the 4th birthday)

  • 2 MMR (measles, mumps, rubella)

  • 2 Varicella (chickenpox) or reliable history of the disease

  • 1 Tdap (tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis)

  • 1 Meningococcal Conjugate Vaccine (MCV4)

    • A second MCV4 is required for entry into Grade 12.

    • Only one dose is required if the 1st dose is given on or after 16th birthday

3. Each immunization entry must include:

  • Vaccine type, date administered, and the name of the provider

Vaccine requirements for Maine students

On September 1, 2021, based on the new law LD 798 all students enrolled in grades PreK through 12 must be immunized against diphtheria, tetanus, pertussis, measles, mumps, rubella, polio, and varicella. Additionally, all students enrolled in grades 7-12 must be immunized against meningococcal disease.

LD 798 removes both philosophical exemptions and religious exemptions from the exceptions to immunization requirements.

Medical exemptions are allowed if you provide a written statement from a licensed physician, nurse practitioner, or physician assistant that, in the practitioner's professional judgment, immunization against one or more of the diseases may be medically inadvisable.

Philosophical/Religious Exemptions for Students with IEPs are allowed if the student elected the philosophical or religious exemption on or before September 1, 2021. In this case, in order to attend school, the following criteria must be met:

a.) The parent or guardian of the student provides a statement from a licensed physician, nurse practitioner, or physician assistant stating that the practitioner has consulted with that parent or guardian and has made the parent or guardian aware of the risks and benefits associated with the choice to immunize

b.) If the student is 18 years of age or older, the student provides a statement froma licensed physician, nurse practitioner, or physician assistant that the practitioner has consulted with that student and has made that student aware of the risks and benefits associated with the choice to immunize.
